Click tracking links show my server's raw domain instead of a clean redirect

Links in my sent emails are showing as something like http://myserver-ip-address.com/click/abc123 instead of using my configured tracking domain. I did set up a custom tracking domain and it shows as verified. Why would it still be using the raw server address instead?
